2024-25 Incident Reporting Form
Please use this form to report incidents such as missing or late officials or timekeeper, facility issues (dirty dressing room, damaged item, etc.), serious incidents between players, team staff and/or spectators, or serious injuries (i.e., if an ambulance is called, or if a player will be out for several weeks or longer).

Incident Details

Please provide information about the incident, including interaction with other associations/ entities such as arena staff, GHRA officials/ timekeeper, EMS, staff from opposing team. Do not provide specific names, but if needed refer to position. You will be contacted if we need more information.
