2013-14 Rep Team Staff Form or
2013-14 House League Staff Information Form if you have not already done so. Be sure to include the expiry date/ date received of any certifications, and indicate if you are intending to obtain any re-certification this season.
We are collecting this information to fulfill the new mandated volunteer screening requirements, and to help us budget for team staff training costs.
Note that new or re-certifications must be obtained by Oct. 31, and reimbursement forms submitted by Dec. 1. The GGHA reimburses 100% of Speak Out, and 50% of required coach or trainer clinics.
